Online Spring Student Evaluation Instructions

  • Annual Evaluations are required for ALL students except for students currently on internship.
  • Missing or incomplete evaluations will lead to an academic hold, blocking registration at UC San Diego.

NOTE: If you are unable to connect to the UC San Diego online evaluation, it is most likely because you are not on a Virtual Private Network (VPN) from your home/coffee shop. If you experience problems logging into the Grad Division website, you will need to first VPN into campus. For directions, visit the “Virtual Private Network (VPN) at UCSD” webpage.

For Students:

  1. Schedule a formal meeting with your Guidance Committee to assess your progress before the end of UC San Diego Spring Quarter
  2. Preparing For Your Committee Meeting: You should prepare a summary of your progress in the past year to distribute to the Committee at the beginning of the meeting. At minimum that summary should consist of:
    • an updated CV that includes any publications, presentations, awards, or grants obtained in the past year
    • for students still completing coursework, a summary of all courses taken, including the grade received in each
    • future courses and other milestones yet to be completed and the timeline and anticipated date of completion for each of the remaining degree requirements

Completing the Online Spring Evaluation:

You will receive detailed instructions on how to complete the online Spring Student Evaluation from the UC San Diego Program Coordinator once the system opens (by the end of March each year).

Note: You will be able to view or download a pdf of your evaluation at any time. You can login to UC San Diego’s online evaluation site at https://gradforms.ucsd.edu/portal/student/index.php (click on the “Spring Evaluation” button and log in using your Single Sign-On account).

For Student Mentors/Committee Chairs:

Mentors/Guidance Committee Chairs are required to complete all fields and sign the online Spring Evaluation (when prompted by email) for your student(s).

Note: You will be able to view the evaluation(s) for your student(s) at any time. You can login to UC San Diego’s online evaluation site at https://grad.ucsd.edu/academics/index.html (go to “Faculty Tools” then click on the appropriate box).

  • SDSU Faculty – You can access this site using your affiliate account. Please contact the UC San Diego Program Coordinator if you need help accessing the site.

For Guidance Committee Members:

Guidance Committee Members are required to sign the online Spring Evaluation (when prompted by email) for students who have advanced to candidacy only. You may log in and view the online Spring Evaluation for your non-advanced students at any time.

For students who have advanced to candidacy: Once the evaluation has been completed and signed by the Mentor/Guidance Committee Chair, then viewed by our staff, you will receive an automated email instructing you to login and sign the evaluation.

Note: You will be able to view the evaluation(s) for ALL of your student(s) (including those who have not yet been advanced to candidacy) at any time. You can login to UC San Diego’s online evaluation site at https://grad.ucsd.edu/academics/index.html (go to “Faculty Tools” then click on the appropriate box).

  • SDSU Faculty – You can access this site using your affiliate account. Please contact the UC San Diego Program Coordinator if you need help accessing the site.
